FDA Clears Motivista, a Smartphone App Designed to Treat Schizophrenia Symptoms

The U.S. Food and Drug Administration has granted clearance to Motivista, a smartphone-based digital therapeutic designed to reduce negative symptoms of schizophrenia, including social withdrawal, reduced motivation, and emotional flatness.

The clearance marks the first time the FDA has approved a digital therapeutic specifically targeting negative symptoms of schizophrenia, which are often harder to treat than the hallucinations and delusions associated with the condition. Existing antipsychotic medications are generally more effective against positive symptoms.

Motivista works through structured daily exercises delivered via a smartphone app, drawing on cognitive behavioral therapy and behavioral activation techniques. Clinical trials showed that patients who used the app alongside their standard medication had greater improvements in negative symptoms compared to those who received medication alone.

The app is intended to be used as an adjunct to existing treatment, not as a replacement for medication or in-person therapy. Developers said it was designed to be accessible to patients who have limited access to mental health specialists, particularly in rural areas.

Psychiatrists and mental health advocates said the clearance was a meaningful step, though they cautioned that digital therapeutics require patient engagement to be effective and that some people with schizophrenia may struggle to use a smartphone app consistently.

The company behind Motivista said it plans to work with insurers and Medicaid programs to make the app available at low or no cost to patients. Pricing and coverage details were not announced at the time of clearance.
