A centuries-old heart drug is getting a second look after new research found it may reduce hospitalizations and worsening events in heart failure patients.

A meta-analysis presented at the 2026 Heart Failure Association congress analyzed data from more than 9,000 patients across three major trials: DECISION, DIGIT-HF, and DIG. Researchers found that treatment with digitalis glycosides, the drug class that includes digoxin, was associated with a 15 percent reduction in the risk of cardiovascular death or first worsening heart failure event.

The benefit was driven primarily by a 25 percent reduction in the time to the first worsening heart failure event.

The findings came alongside results from the DECISION trial, a double-blind, placebo-controlled study of 1,001 patients with chronic heart failure. That trial did not meet its primary endpoint, finding no statistically significant reduction in the composite of worsening heart failure events and cardiovascular mortality. However, the broader meta-analysis suggested a meaningful clinical benefit when data from multiple trials were combined.

Researchers noted that digoxin did not show a statistically significant reduction in mortality on its own. They also found that patients who stopped taking digoxin experienced more clinical deterioration than those who stopped taking a placebo, suggesting the drug provides ongoing benefit.

Digoxin requires careful dosing and monitoring of serum levels to avoid toxicity, which can cause nausea, vomiting, and dangerous heart rhythm problems. Experts said patients should not start, stop, or adjust their dosage without consulting a doctor.

The drug is inexpensive and widely available. Researchers said it may serve as a useful addition to existing guideline-directed therapy for selected heart failure patients who are already on standard treatments.
