The study tracked thousands of adults over several decades, examining how cardiovascular risk factors in midlife affected cognitive decline in later years. Researchers found that people who controlled all three risk factors during their 40s, 50s, and early 60s showed significantly later onset of dementia compared to those who did not.
The 13-year delay is substantial. Dementia typically begins to affect people in their 70s and 80s. Pushing that onset back by more than a decade would allow many patients to live independently for longer and reduce the burden on caregivers and healthcare systems.
"These are not exotic interventions," said one of the study's lead researchers. "Blood pressure medication, diabetes management, and not smoking are things we already know how to do. The question is whether people do them consistently."
High blood pressure damages blood vessels throughout the body, including those in the brain. Diabetes causes inflammation and vascular damage that accelerates cognitive decline. Smoking compounds both effects.
The research adds to a growing body of evidence that dementia is not inevitable and that lifestyle choices made decades before symptoms appear can significantly affect outcomes.
Public health experts say the findings should prompt more aggressive screening and treatment of cardiovascular risk factors in people in their 40s and 50s, a period when many people feel healthy and may not prioritize preventive care.
The study was published in a peer-reviewed journal and presented at an international neurology conference in August 2026.
